Breathe, Inc. Grant Programs

To apply for our Breathe, Inc. Grant Program, please follow these directions:

(1) Email us with your contact information and a brief explanation as to what services you are interested in. We will then contact you and complete a brief assessment before moving on with the grant process. Prior to completing the application please contact us at: Shannon@breatheinc.org

You can apply for the Breathe, Inc. grant program if you are a cancer patient receiving active treatment. Grant amounts are subject to availability.

(2) Once the assessment is completed, please print, complete, and mail our application to Breathe, Inc. Applications can be mailed to:

Financial Assistance

for Walworth County, WI Residents in Need.

Due to availability of funds we are currently concentrating on patients residing in Walworth County, WI, or receiving active treatment in Walworth County, WI. Active treatment does NOT include surveillance or hormone therapy. Grant amounts are subject to availability and can be used for rent/mortgage, utility payments (water, electric, and sewer), phone/cable/internet bills, car insurance, health premiums, or car payments.

Breathe Inc. will make check(s) out directly to the company you are seeking assistance for. The check(s) will be mailed to you so you can add your account number and forward to the company.

Be sure to continue to pay your bills as applications are reviewed monthly with check(s) being mailed out at the end of each month.

All checks must clear the bank within 60 days. Any outstanding checks beyond the 60 days will be canceled and applicants must reapply for assistance.

Although grants may not be available at this time, we are happy to be your advocate assisting with healthcare navigation and resources.

Working with Breathe, Inc. you will be partnered with a client specialist that works closely with you to develop a plan for assistance and to give you relief and support.

Customized Support Includes:

We listen to you and work with you to devise a supportive plan
We give guidance where you need it
We refer and apply for resources on your behalf
There is no runaround, or “transferring you” to someone else after you’ve told us about the hardships you are facing.
We want to offer you help; however you need it, meeting you where you’re at, and supporting you in the ways you need support.
